18 MONTHS, Grant-Funded Position
Salary: $45,000 per year
Duties: Reports to Director of Re-entry. Services a caseload of 25-30 inmates. Provides individual case management; develops comprehensive wrap around re-entry plans; provides or brokers services to holistically address inmates’ needs; documents all case management activities in a database; supervises two Community Aids, leads team case management meetings; perform other related duties as required.
Requirements: Graduation from an accredited college or university with a Bachelor’s degree. Ability to effectively read, write, speak and understand English. Bilingual preferred. Possess an understanding of computers and the Internet. Proficiency in utilizing a database for case management. Familiarity working with incarcerated individuals.
Hours: Full-time, 40 hours per week. Some evenings and weekends required.
Note: Offers of employment for this title are contingent upon successful completion of physical fitness testing, to include drug screenings, and criminal background checks.
