You are hereby notified of the below listed clarification(s):
Item 1: Dr
. Kenneth V. Master is currently providing these services.
Item 2: Current retainer
is $500.00 per month.
Item 3: Services are currently provided on
-site one (1) full day per month; approximately 15 - 20 residents are seen that day.
Item 4:
Proposals for provision of these services will be issued again next year.
The date
/time originally established for receiving sealed proposal submissions, June 30, 2010, at 1 :00 PM, remains unchanged.
