Advanced Healthcare Directives
The New Jersey Legislature, in 1991, passed Legislation for Health Care Directives and Durable Powers of Attorney, commonly known as a Living Will. The New Jersey Bioethics Commission drafted the Health Care Directives in understandable language. Part of their mission was to see that these forms were distributed State wide. The Commission asked the Surrogates of the State of New Jersey to assist in the education of the public by making forms available to anyone who is interested.
They are presently available upon request in the Surrogate's office at no cost. The Surrogate has spoken to various clubs and church groups for the past seven years, and made the forms available. The Surrogate is available to any group that is interested in having him speak to explain these forms. |
