Choose your date and time and answer the appropriate questions.
You will receive the location of the vaccination clinic site during the
registration process. Once completed, you will receive a conformation
email that will have a link to the “Covid-19 Vaccination Registration Form”.
This is a fillable form. Please complete the form, print it out, and bring it
with you to your appointment. Please note: if you go to register and there are
no time slots showing, please close the browser window and try to sign in
again.
